NOTICE OF PRIVACY PRACTICES

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who this notice covers

This notice applies to Remmers Dental, 8250 Watterson Trail, Louisville, KY 40299, and to the dentists, hygienists and staff who work here. It covers your dental records and the billing records that go with them. How this website handles what you type into it is described separately in our privacy policy.

How we use and share your health information

We may use and share your health information without asking for separate written permission for these purposes:

  • Treatment. To care for you, and to work with other dentists, specialists, laboratories and physicians involved in your care.
  • Payment. To bill you, your insurance plan, or another person responsible for your account, and to check your benefits.
  • Running the practice. To schedule and remind you of appointments, review the quality of our care, train staff, and work with companies that help us do these things under a written agreement to protect your information.

We may also share information when the law requires or permits it: to public health authorities, to report suspected abuse or neglect, to health oversight agencies, in response to a court order or lawful subpoena, to law enforcement in limited circumstances, to prevent a serious threat to health or safety, to coroners and funeral directors, for workers' compensation, and for certain government functions.

Family, friends, and reminders

If you agree, or do not object when given the chance, we may share information with a family member or another person who is involved in your care or helps pay for it. We may contact you with appointment reminders by phone, mail, email or text, using the contact details you gave us. Tell us if you want to be reached a different way.

When we need your written permission

We will ask for your written permission before we use or share your health information for marketing, before any sale of your information, and for any purpose this notice does not describe. You may take your permission back in writing at any time, and we will stop from that point on.

Some records have extra protection under federal or Kentucky law, including records from a substance use disorder treatment program. We follow the stricter rule wherever one applies, and such records are not used or shared in legal proceedings against you without your written consent or a court order.

Your rights

You have the right to:

  • See and get a copy of your dental and billing records, on paper or electronically. We will provide it, usually within 30 days, and may charge a reasonable, cost-based fee.
  • Ask us to correct a record you believe is wrong or incomplete. If we say no, we will tell you why in writing within 60 days.
  • Ask us to contact you privately, for example at a particular phone number or address. We will agree to reasonable requests.
  • Ask us to limit what we use or share. We are not required to agree to every request, but if you pay for a service in full yourself, you can ask us not to share it with your insurance plan, and we will agree unless a law requires otherwise.
  • Get a list of those we have shared your information with in the past six years, other than for treatment, payment, running the practice, and disclosures you asked for.
  • Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you. A person with your medical power of attorney, or your legal guardian, can use these rights on your behalf.

To use any of these rights, call the office or write to us at the address below.

Our responsibilities

The law requires us to keep your health information private and secure, to give you this notice, and to follow the notice currently in effect. If a breach affects the privacy or security of your information, we will tell you promptly.

We may change this notice, and the change will apply to all the information we hold about you. The current notice is always on this page and available at the front desk.
